Basic Qualifications

Education/Training:  Graduate of a school of nursing with a current license(s) to practice as a nurse in the state(s) served by SpiriTrust Lutheran Home Care & Hospice.  Current CPR certification within three (3) months of employment.

Skill(s):  Proficient reading, writing and grammatical skills; proficient in interpersonal relations and communication skills, including pleasant telephone manner and speaking voice; ability to work independently and to establish and maintain effective working relationships; working knowledge of computer operations and ability to learn specialized industry applications; knowledge of currently acceptable practice; effective leadership and supervisory skills; valid driver’s license and dependable means of transportation.

Experience:  A minimum of two (2) years of experience in a community health agency normally required. 

General Responsibilities

Responsible for assisting clinical supervisors with coordinating care provided to designated clients; communicating with appropriate management and staff personnel; providing periodic reports to clinical supervisors; participating with the Director in establishing specific goals for the department; coordinating work within the department, as well as with other departments; directly supervising assigned personnel; communicating with appropriate management and staff personnel; providing periodic reports.

Essential Duties

Supports the mission of SpiriTrust Lutheran Home Care & Hospice, which is to answer Christ’s call by providing health care and related services to those who are striving to achieve their highest possible level of independence, quality of life, health and comfort. 
Supports the mission of SpiriTrust Lutheran, which is witness to the Gospel of Jesus Christ, providing exceptional health, human and senior living services by supporting persons in achieving an optimal quality of life.
Coordinates care provided to designated clients of which the following are illustrative:

Facilitates and coordinates IDG in conjunction with the clinical supervisors.

Coordinates placement and supervision requirements of hospice aides in cooperation with Team Assistant and visiting staff.

Assists nurse in the development, implementation and evaluation of nursing regimen for designated group of clients.

Assists in identifying client needs and recommends appropriate intra/inter agency referrals.

Assists in the development of the plan of care in consultation with the assessing staff.

Assures adequate exchange of information and facilitates communication and interaction among personnel and agencies providing services as follows:

Coordinating patient care by all service disciplines in all care settings.
Functions as liaison between all disciplines and agencies providing care.
Facilitates communication between caregivers and families.

Assists Team members to determine priorities and frequencies of client visits.

Identifies and leads the Best Practice implementation

Assures clinical competence at bedside for  patient satisfaction, point of care documentation, HIS, comprehensive assessment (including NTFC) by making observational visits with staff at a level determined by the Director, Patient Access

Manages client and agency activities as follows:

Participates in quality improvement processes and activities.

Assumes assigned duties of Clinical Supervisor in his/her absence.

Runs daily reports as assigned by Director of Patient Access

Runs weekly IDG agenda and distributes to clinical supervisors

Participates in agency on-call as needs dictate.

Provides direct care when special skills, teaching or assessment are needed or agency needs mandate as follows:

Travels to the client’s location in various weather conditions.

Gains access to the client by available means which may include but is not limited to steps; uneven surfaces; wet, snow covered or icy surfaces; narrow entrances; etc.

Observes visual, olfactory, tactile and auditory reactions of client’s illness and reports same.

Communicates with clients to identify symptoms and reactions of client’s illness and reports same.

Implements planned interventions as follows:

Performs procedures and techniques correctly and safely with the resources available at the client’s location.
Executes medical regimen.
Moves or assists with the movement of adult and pediatric clients with or without assistance as available.

Participates in evaluation of care plan.

Directly supervises assigned personnel as follows:

Assists in the selection of new personnel as appropriate.

Makes provisions for the proper orientation and training of new personnel.

Reviews employee performance throughout the probationary period and on a regularly scheduled basis thereafter.

Organizes, schedules and distributes work among assigned personnel.

Keeps personnel informed of pertinent policies and procedures affecting the department and/or their jobs; creates an atmosphere in which upward communication from employees is encouraged.

Administers personnel policies and procedures as established by organization policy.

May act as Risk Control Nurse as assigned as follows:

Evaluates the working environment to reduce the risk of harm to human and material resources.
Maintains awareness of federal and state regulatory issues as they relate to risk and interprets these issues.
Participates in the development and implementation of emergency response plan.
Assists employees/volunteers to identify risks and to take proper precautions including administering Hepatitis B Vaccine, pneumovax and Mantoux testing according to agency policy and Influenza vaccine.
Assures proper disposal of hazardous materials and keeps Hazardous Substance List up-to-date.
Serves as consultant, resource person and pre/post test counselor for post exposure to bloodborne pathogens for clients and employees of SpiriTrust Lutheran Home Care & Hospice including home visits as necessary.
Administers Mantoux and Drug testing for new personnel, interprets results and assures appropriate follow-up.
Maintains a system for client incident/accident reporting and infection control surveillance, including investigation and corrective action; reports collected data and recommendations for review by the Risk Committee.
Works with the Education department to develop, coordinate and present training programs which address proficiency and safe practices and promote safety consciousness.

Leads and models appropriate adherence to all internal policies, procedures, practices, laws and regulatory requirements and ensures that all personnel adhere to the same.
Cooperates with, participates in and supports the adherence to all internal; policies, procedures and practices, laws and regulatory requirements.
Works harmoniously with others.
Coordinates specific work tasks with other personnel within the department as well as with other departments in order to ensure the smooth and efficient flow of information.
Communicates with his/her supervisor, other supervisors, community relations coordinators and appropriate staff in order to integrate activities.
Responds to inquiries relating to his/her particular area, or to requests from other personnel, clients, etc. within given time frames and within established policy.
Maintains appropriate records and provides assigned reports.
Supports and advances the Lean philosophy in all endeavors.
Participates on project teams and may perform other duties as assigned.
