Education/Training: Must be a graduate of an applicable bachelor’s or master’s degree program. Registered Nurse or Social Worker degree preferred, or possesses a suitable combination of education and experience. Must attain and maintain CPR certification.
Skill(s): Able to speak, read, write and understand English; proficient at written and oral communication; proficient at basic math skills; effective interpersonal and communication skills; strong critical thinking and decision making skills; proficient computer skills; valid driver’s license and dependable means of transportation.
Experience: Five years of applicable experience with at least two years of supervisory experience and one year of experience working with a frail or elderly population required.
Eligibility for Employment: Must meet eligibility requirements to work in a Medicare/Medicaid environment including criminal background and other eligibility clearances, clearance for communicable diseases and all required immunization must be current.
Responsible for the planning, coordination and implementation of all activities of the LIFE center, including supervision of staff and assurance of appropriate participant care and caregiver support; rendering care within their credentialed scope of practice; managing agency activities and material resources; implementing strategies to achieve goals developed for the assigned region as part of the annual operating plan and budget; participating in budget development coordinating work within the department, as well as with other departments; ensuring compliance with operating policies and procedures and outside regulatory requirements; directly supervising assigned personnel; communicating with appropriate management and staff personnel; providing periodic reports.

Coordinates interdisciplinary care team’s care management process:
Provides leadership and facilitates the interdisciplinary care team meetings for development and implementation of care plans.
Ensures that documentation of team meetings is maintained and changes are recorded in individual care plans.
Ensures that notification is sent to appropriate agencies and regulatory bodies when required.
Meets regularly with participants, family and/or caregivers, and other appropriate persons to explain and interpret standards and procedures, find solutions to problems, and to ensure that the care being given is consistent with the well-being of the participants.
Ensures monitoring of participants’ progress and decline and that changes are communicated to applicable interdisciplinary team members and to appropriate family and/or caregivers.
Provides written notice to participants/representatives of partially or wholly denied service requests and the participant’s appeal rights.
Ensures the continued development of the team to improve the decision making process.
Evaluates and improves performance by working within the Quality Assurance and Performance Improvement (QAPI) program.
Ensures documentation in medical records is complete and current.
Ensures caregivers are provided with effective support in order to produce the best possible outcomes for participants.
Thinks creatively to produce solutions to participant issues that enable participants to function safely with the highest possible level of independence.
Manages the operations of the center and material resources as follows:
Ensures that the operations of activities, nutrition, personal care, clinical services, rehabilitation and other services of the center are within the standards and regulations of the program and are designed to meet the needs or participants.
Ensures participants are initially assessed and oriented to the program and are actively involved in the plan of care development and are free to express their opinions about the care and services they receive.
Ensures staff are appropriately licensed and in-service requirements are met.
Inspects the physical environment and plant operation on a regular basis; takes necessary steps to ensure participants are provided a clean, healthful and “home-like” atmosphere and the center is in compliance with regulatory requirements.
Participates in the development, interpretation, implementation and evaluation of policies and procedures.
Participates in Performance & Quality Improvement activities and assures implementation of approved corrective actions.
Prioritizes the needs of agency and allocates staff to meet unscheduled and scheduled demands.
Implements strategies to achieve goals assigned to the department as established in the annual operating plan; assists in the development of the annual budget for the department and adheres to budget parameters.
Delegates duties to qualified staff as appropriate.
Assures coverage of essential duties during planned and unplanned absences of staff.
Assures compliance with reimbursement requirements.
Directs and supervises staff as illustrated by the following:
Recruits, selects and orients staff; regularly reviews the competence and performance of this group.
Makes certain that all relevant information is properly communicated to the staff in order to give them every opportunity to perform their duties properly.
Conducts regular meetings with the staff.
Works closely with the staff in implementing an effective human resource management program to ensure a competent, motivated group of employees.
Administers established policies and procedures as established by the Agency.
Attends and actively participates in Management Team and other committees as assigned.
Participates in the 24-hour care delivery and on call rotation as needed.
Reports pertinent information to the immediate supervisor
Completes and submits reports accurately and on time including, but not limited to, organizational reports and reports to CMS, OHS, NPA, PALPA, etc.
Interacts with participants, caregivers, staff, vendors, other SpiriTrust Lutheran program staff and all stakeholders and the general public in a courteous, respectful, supportive and service oriented manner befitting a faith centered organization.
Assures personal and departmental compliance with organizational policies and procedures and local, state and federal regulations for the operation of a PACE program and Adult Day Care Center including, but not limited to:
Fire drill response
Reporting employee injuries and security incidents
Identification, prevention and reporting actual or suspected abuse, neglect, exploitation and improper use of chemical and physical restraints
Participant Bill of Rights
Participant complaints and grievances
Safety and infection control
Medical clearance and required immunizations
Meets competency requirements and acts within scope of practice and with appropriate supervision at all times.
Coordinates tasks with other personnel in order to ensure the smooth and efficient flow of information.
