 General Responsibilities

Participates as a member of the corporate Senior Living Team supporting the Senior Living communities, responsibilities include:

Organizing and managing the clinical components of the electronic health record activities.
Provide basic and on-going electronic health record orientation/education to Senior Living Communities and perform needs analysis/phase in process as deemed necessary. 
Provide clinical systems guidance and support in conjunction with the Information Technology team. Initiates, and guides implementation of clinical excellence special projects.
Support clinical oversight and regulatory compliance for assigned communities/programs. 
Lead and mentor community/program clinical leader through coordinating and monitoring clinical practices and outcomes, standards of practice, care delivery, documentation to support care and reimbursement.

  
Basic Qualifications

Education/Training:  Current unencumbered RN license for the Commonwealth of Pennsylvania required. Bachelor’s Degree in nursing, healthcare administration, public health policy, or related clinical health field preferred.

Skill(s): Speak and understand English; proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communication skills; proficient PC skills; proficient working knowledge of epidemiology, infectious diseases, and aseptic technique to include Universal Precautions; proficient teaching and staff development skills; and valid driver’s license.  Refer to separate analysis for physical requirements of the essential functions (with or without accommodation).

Experience:

A minimum of three years’ experience in one or more of the following fields:  long-term care nursing, clinical information management, and/or healthcare administration. A minimum of one year experience in implementation of an electronic health record.  Experience in staff development preferred.

Essential Duties

Interacts with residents, clients, staff, and the general public in ways that demonstrate the agency’s commitment to courtesy, service, and hospitality befitting a church related organization
Responsible for providing clinical oversight and regulatory support, to lead and mentor community/program clinical leaders by:
Maintains current knowledge of standards of practice, and federal, state, and local regulations affecting skilled and personal care levels of care
Monitors clinical practice and outcomes through routine monthly visits utilizing visit tools, process monitors, and nursing rounds to observe care delivery.
Monitors program compliance with infection control reporting, clinical processes, care delivery, and agency standards.
In conjunction with program leadership, analyzes trends, identifies high-risk problem areas, and analyzes clinical outcome data; monitors and analyzes program incident reports, adverse event reporting to DOH/DPW, complaint logs.  Makes recommendations and monitors compliance to corrective actions to assure progress made to rectify concerns noted.
Recommends and supports implementation of procedures to address adverse outcomes.
In conjunction with program leaders, monitors and analyzes outcomes, develops and implements clinical strategies and care pathways to improve outcomes.
Provides on-the-spot and larger group education to all levels of staff, as needed.
Monitors and coordinates Performance Improvement Plans for identified outcomes.  Supports and monitors program committees including QAPI/Quality Management, Infection Control, Rehabilitation as  applicable to program.
Collaborates with program staff to ensure state and federal (MDS) documentation is appropriate and accurately reflects care that is given through random chart audits and observation of care delivery.
Conducts program survey readiness monitoring through routine visits and participation in the peer review mock survey process.
Monitors MDS accuracy, care plan development and implementation.
Monitors electronic documentation
Participates in developing and authorizing educational programs and presentations.
Maintains ongoing review of all SPIRITRUST LUTHERAN and regulatory standards and procedures to ensure program compliance
Provides educational support for skills development, regulatory deficiencies, new policy implementation, regulatory changes, and other clinical findings.
Monitors infection control practices.
Provides clinical oversight for ongoing survey readiness, clinical delivery, and system support as illustrated by the following:
Assists with the development of standards of practice and procedures.
Provides support to the programs during peer review mock survey.
Provides support in the development and monitors compliance of action plans for Department of Health/ Department of Public Welfare surveys reporting and Peer Review surveys
Provides support to the programs during the Department of Health and surveys.
Monitors plans of correction from Department of Health and Department of Public Welfare to ensure compliance with established standards.
Monitors the implementation of clinical processes and ensures continued compliance is maintained.
Monitors the programs’ adherence to State and Federal guidelines relating to abuse prevention as follows:
Monitors and evaluates the effectiveness of the programs’ investigation process.
Provides support and guidance during investigations as needed
Performs random audits on incident and accident reports.
Provides ongoing support and in-servicing to all staff involved in investigations.
Serves as the organization’s electronic health record(E.H.R.) resource for staff  as follows:
Maintains proficiency/ teaching knowledge of the E.H.R. platform for the purposes of staff education and in-servicing
Assists in the maintenance of an agency wide E.H.R. in conjunction with the Information Technology team.
Coordinates specific work tasks with other staff within the department as well as with other departments in order to ensure the smooth and efficient flow of information.
Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the work place.
Cooperates with, participates in, and supports the adherence to all Agency standards, procedures, and practices in support of electronic health record and electronic orientation/education and overall soundness and compliance with all regulatory requirements, i.e., HIPAA, OSHA, DPW, OIG, and Home Care Licensure etc.
Abides by and supports the organization’s code of conduct by exhibiting behavior that is set forth in the code of conduct.  Performs duties in accordance with the code of conduct and completes annual compliance training.
Reports pertinent information to the immediate supervisor as requested, or according to an established schedule; compiles information as necessary or as directed and provides data to appropriate personnel.
Responds to inquiries relating to his/her particular area, or to requests from customers, other personnel, etc., within given timeframes and within established policy.
