Basic Qualifications


Education/Training:  Must be a graduate of an accredited school of nursing and possess a valid Pennsylvania RN License.  Must have valid driver’s license.  Must attain and maintain CPR certification.


Skill(s):  Able to speak, read, write and understand English; proficient at written and oral communication; proficient at basic math skills; effective interpersonal and communication skills; moderate computer skills; valid driver’s license and dependable means of transportation.


Experience:  Three (3) years of RN experience including one year’s experience working with a frail or elderly population.  One year’s supervisory experience required.


General Responsibilities


Responsible for Oversight of the day to day operations of the Clinics in both LIFE Centers.  Responsible to promote and maintain the health of participants through nursing assessment, treatment, teaching and counseling; participates in enrollment and periodic assessments of participant health needs to contribute to plans of care; manages complex clinical situations for which he/she is responsible; managing agency activities and material resources; implementing strategies to achieve goals developed for the assigned region as part of the annual operating plan and budget; participating in budget development coordinating work within the department, as well as with other departments; ensuring compliance with operating policies and procedures and outside regulatory requirements; directly supervising assigned personnel; communicating with appropriate management and staff personnel; providing periodic reports.



Essential Duties


Supports the mission, vision and values of SpiriTrust Lutheran and SpiriTrust Lutheran LIFE.  LIFE’s mission is to witness to the gospel of Jesus Christ by providing comprehensive health care and social support to maximize the quality of life for older adults in the communities we serve.


Manages the operation of the clinic and assures effective and appropriate care for participants.


Ensures assessments are completed appropriately and on time.
Participates in the development of a plan of care for participant.
Implements, manages and evaluates care.
Utilizes the nursing process and collaborates with interdisciplinary team.
Performs advance nursing assessment using critical thinking skills.
Identifies participant’s overt problems and needs and sets priorities.
Interprets and records latest diagnostic results and forwards critical results to MD or CRNP.
Includes the participant and family in developing or revising the individualized plan(s) of care.
Makes referrals appropriately.
Ensures the care provided conforms to accepted practice standards.
Provides correct treatments, procedures and other care as prescribed and according to care standards.

Demonstrates understanding of age-related characteristics and needs of participants served.
Explains nursing procedures in appropriate forms.
Provides education for participants and caregivers.
Identifies emergency situations and independently initiates advanced therapy.
Assures pharmacy and other involved providers are kept informed of participant changes.
Documents participant care delivered as planned and any variation, with appropriate rationale.
Makes and records observations related to impending or associated problems.

Collects participant data and completes required forms with appropriate response according to standard. 


Understands and demonstrates respect for participant rights and utilizes established mechanisms for management of ethical issues in care.


Performs all duties of the Risk Manager for both LIFE Centers.


Reviews clinical practice related participant incidents and/or injuries; makes referrals to the Medical Director as indicated; identifies and reviews problems or potential problems with direct or indirect negative quality of care implications.
Assures proper disposal of hazardous materials and assures Hazardous Substance List is up-to-date.
Evaluates the working environment to reduce the risk of harm to human and material resources.
Maintains awareness of federal and state regulatory issues as they relate to risk and interprets these issues.
Participates in the development and implementation of emergency response plans.
Serves as a consultant, resource person and pre/post test counselor for post exposure to bloodborne pathogens for participants and employees of SpiriTrust Lutheran LIFE including home visits as necessary.
Assures the maintenance of a system for incident/accident reporting, participant and employee infection control surveillance including investigation and corrective action, reports collected data and recommendations for review by the Safety Committee for both LIFE Centers.
Responsible for oversight of the Safety Program at LIFE
Reviews and monitors employee incident reports to identify potential risks; investigates and makes recommendations concerning changes in related policy and/or procedure.
Assists employees/volunteers to identify risks and to take proper precautions including administering Hepatitis B Vaccine, pneumovax and Mantoux testing according to agency policy, influenza vaccine.
Administers Mantoux and Drug testing for new personnel, interprets results and assures appropriate follow-up.
Reviews medical reports for new personnel and those receiving medical treatment which may affect fitness for duty.
Works with the Education Department to develop, coordinate and present training programs which address proficiency and safe practices and promote safety consciousness.
Serves as liaison in coordinating Worker’s Compensation cases.
Responsible for infection control monitoring, reporting and quarterly in-services for both LIFE Centers.


Performs all QAPI duties assigned to the Clinic for both sites including but not limited to audits, reporting and completion of action plans when trends identified.


Provides leadership


Using nursing process, delegates nursing care to appropriate personnel.
Integrates cost effective measures into nursing practice.
Recognizes problems and takes responsibility for documenting same.
Demonstrates active participation in quality assurance and performance improvement processes.
Complies with program expectations to meet staffing demands based on participant care needs.
Complies with policies addressing safe working conditions.
Identifies and corrects unsafe working conditions or practices.
Recognizes inappropriate and/or ineffective participant care management, resolves issues and problems, completes written reports and documents issues in EMR.


Directly supervises assigned personnel as follows:


Recruits, selects, orients and makes provisions for the proper training of staff; regularly reviews the competence and performance of this group.
Makes certain that all relevant information is properly communicated to the group in order to give them every opportunity to perform their duties properly.
Conducts regular meetings with the staff.
Works closely with the group in implementing an effective human resources management program to ensure a competent, motivated group of employees.
Administers Human Resources Standards of Practice and other procedures as established by agency policy.
Works with supervised employees to ensure work schedules are developed to meet program/center needs.


Manages agency activities and material resources as follows:


Participates in the development, interpretation, implementation and evaluation of policies and procedures.
Participates in Performance & Quality Improvement activities and assures implementation of approved corrective actions.
Prioritizes the needs of agency and allocates staff to meet unscheduled and scheduled demands.
Implements strategies to achieve goals assigned to the department as established in the annual operating plan; assists in the development of the annual budget for the department and adheres to budget parameters.
Delegates duties to qualified staff as appropriate.


Participates in 24 hour on-call rotation.


Reports pertinent information to the immediate supervisor.


Completes and submits reports accurately and on time including, but not limited to, organizational reports and reports to CMS, DHS, NPA, OSHA, PALPA, etc.


Interacts with participants, caregivers, staff, vendors, other SpiriTrust Lutheran program staff and all stakeholders and the general public in a courteous, respectful, supportive and service oriented manner befitting a faith centered organization.


Assures personal and departmental compliance with organizational policies and procedures and regulatory requirements including, but not limited to, local, state and federal regulations for the operation of a PACE program and Adult Day Care Center including, but not limited to:


Fire drill response
Reporting employee injuries and security incidents
Identification, prevention and reporting actual or suspected abuse, neglect, exploitation and improper use of chemical and physical restraints
Participant Bill of Rights
Participant complaints and grievances
Safety and infection control
Medical clearance and required immunizations.


Meets competency requirements and acts within scope of practice at all times.


Coordinates tasks with other personnel in order to ensure the smooth and efficient flow of information.
