Education/Training: Master’s degree in social work or social welfare or current social work license from the Commonwealth of Pennsylvania required.
Skill(s):Speak and understand English; proficient reading, writing, grammar, and mathematics skills; excellent interpersonal relations and communication skills; proficient organizational skills; moderate PC skills; knowledge of general medical terminology; demonstrated management and supervisory skills; valid driver’s license when required to meet Program’s needs.
Experience:A minimum of three (3) years’ experience in related positions normally required, including one (1) year of supervised social work experience in a health care setting working directly with individuals.
Responsible for planning, organizing, and directing the social services function; providing oversight to the resident admission process to the Skilled Care Center; implementing strategies to achieve goals developed for the department as part of the Community's annual operating plan; insuring the department's compliance with operating standards and procedures and outside regulatory requirements; organizing the work, activities, and human resources of the department; directly supervising assigned personnel; communicating with appropriate management and staff personnel; providing periodic reports.

Interacts with residents, clients, staff, and the general public in ways that demonstrate the agency’s commitment to courtesy, service, and hospitality befitting a church related organization.
Plans, organizes, and directs the social services function as illustrated by the following:
Implements and periodically updates written standards and procedures governing the day-to-day operations of the Department.
Works in conjunction with the center’s Quality Assurance Committee to develop and implement an on-going quality assurance program.
Ensures that the Department is in compliance with all regulatory requirements; takes appropriate actions to respond to and correct any deficiencies resulting from regulatory surveys and inspections.
Develops, implements, and monitors the Social Services Department budget
Assesses residents' social and emotional needs; informs new residents or residents' families of their skilled care center rights and provides information concerning Advance Directives; discusses and documents residents' preferences concerning CPR
Develops a discharge plan for all residents according to established procedures.
Obtains release from residents or responsible party for restraints ordered by physicians.
Audits medical charts periodically for social services documentation accuracy and completeness of required records.
Communicates regularly with residents' families concerning residents' social and emotional needs; maintains an open forum for questions and concerns; discusses counseling needs with residents and residents' families.
Conducts staff education pertaining to Residents Rights as well as any other topic related to the Social Service Department as required or requested.
Addresses room changes with Plan of Care team and recommends actions based and residents' preferences and level of care.
Assists Finance Department in obtaining residents' signatures on financial documents; obtains residents' or responsible party's signature(s) on transfer and discharge forms.
Participates in various committees as assigned.
Provides oversight for the appropriate nursing care of residents in accordance with the Plan of Care as follows
Admission process of prospective residents.
Appropriate and timely clinical information to nursing staff regarding resident admissions.
Nursing assessments process.
Ensures a safe and orderly work environment as follows:
Develops and implements disaster and emergency procedures for the department; assumes a leadership role.
Attends all required safety in-services; participates in all drills.
Provides oversight to the resident admission process to the Skilled Care Center including, but not limited to, the following:
Cultivates a positive working relationship with key members of other health care facilities that refer potential residents.
Cultivates and maintains waiting list.
Conducts interviews with prospective residents and families.
Executes appropriate admission paperwork, documents, and contract.
Orients resident and family as to billing procedures, resident rights, etc.
Oversees team members in their timekeeping process, ensuring accurate submission of data:
Follows up with team members under their supervision to ensure timekeeping process is completed and submitted accurately with no errors.
Educates team members regarding requirements for timekeeping, missed punches, consequences, etc.
Enters department’s timekeeping data accurately and submits data within practice guidelines.
Evaluates data related to resources and program and makes appropriate corrective actions with team members, if necessary, to ensure compliance with all rules, regulations, and agency standards.
Interacts with all colleagues in ways that assist one another in the successful performance of their duties.
Provides support, encouragement, and direction to the individuals under his/her direct supervision; acts as a resource for staff in problem solving and properly communicates within the established chain of command.
Creates an atmosphere that promotes a positive and respectful work environment.
Implements strategies to achieve goals assigned to the department as established in the Community's annual operating plan.
Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the work place.
Identifies, prevents, and reports actual or suspected abuse, neglect, exploitation, and improper use of chemical and physical restraints, in accordance with standards.
Cooperates with, participates in, and supports the adherence to all Agency standards, procedures, and practices in support of risk management and overall safety and soundness and compliance with all regulatory requirements, i.e., HIPAA, OSHA, etc.; insures that the department and all personnel adhere to the same.
Abides by and supports the organization’s code of conduct by exhibiting behavior that is set forth in the code of conduct. Performs duties in accordance with the code of conduct and completes annual compliance training.
Organizes the work and activities of the Social Services Department in order to achieve established goals; monitors the efficiency and performance of the department versus established standards.
Directly supervises assigned personnel as follows:
Recruits, selects, orients, and makes provisions for the proper training; regularly reviews the competence and performance of this group.
Makes certain that all relevant information is properly communicated to the group in order to give them every opportunity to perform their duties properly.
Conducts regular meetings with the staff.
Works closely with the group in implementing an effective human resource management program to insure a competent, motivated group of team members.
Administers HR Standards of Practice and other procedures as established by agency policy.
Communicates with the Skilled Care Center Administrator, other department managers, and appropriate staff personnel in order to integrate goals and activities.
Provides periodic reports to the Skilled Care Center Administrator and other groups as required.

Completes educational requirements as mandated by state and federal agencies and to maintain competencies.
