Basic Qualifications
 
Education/Training:  High School diploma or equivalent.
 
Skill(s):  Proficient reading, writing, typing and grammatical skills; proficient in interpersonal relations and communication skills, including pleasant telephone manner and speaking voice; ability to work independently and to establish and maintain effective working relationships; working knowledge of computer operations and ability to learn specialized industry applications; possesses broad knowledge of community resources generally available; valid driver’s license and dependable means of transportation.
 
Experience:  Minimum of one (1) year of experience in a medical related field is required.
 
General Responsibilities
 
Responsible for serving as the SpiriTrust Lutheran Home Care & Hospice and SpiriTrust Lutheran LIFE representative and resource person for the purpose of receiving, collecting, evaluation and processing referral information and facilitating the referral of potential and existing clients for needed services; coordinating work within the department, as well as with other departments; complying with operating policies and procedures and regulatory requirements; communicating with appropriate personnel; responding to inquiries or requests for information; maintaining appropriate records and providing assigned reports. 

Essential Duties

 

Supports the mission of SpiriTrust Lutheran Home Care & Hospice, which is to answer Christ’s call by providing health care and related services to those who are striving to achieve their highest possible level of independence, quality of life, health and comfort.

 

Supports the mission and values of SpiriTrust Lutheran LIFE, which is to witness to the gospel of Jesus Christ by providing comprehensive health care and social support to maximize the quality of life for older adults in the communities that we serve.

 

Supports the mission of SpiriTrust Lutheran, which is witness to the Gospel of Jesus Christ, providing exceptional health, human and senior living services by supporting persons in achieving an optimal quality of life.

 

Serves as the SLHCH and LIFE representative and resource person for the purpose of receiving, collecting, evaluating and processing referral information of which the following are illustrative:

 


Receives all referrals from a variety of sources and means to include but not be limited to:

 




Handwritten or mechanically produced documents
Telecommunication devices
Internet based communications


 


Assess client needs by listening and asking appropriate questions to understand individual needs.

 


Evaluates referrals in accordance with agency policies, procedures and resources.

 


Acts as an advocate for each client to ensure appropriate direction of care to meet all needs through SpiriTrust Lutheran Home Care & Hospice programs and services as well as outside resources.

 


Provides information to client concerning resources for payment of agency services.  Knows and understands the provisions of payor contracts.

 


Obtains necessary admission information for clients.

 


Follows progress of clients admitted to inpatient facilities to assure smooth transitions to home through communication with inpatient facility staff.

 


Complies with HIPAA regulations regarding gathering, processing and transferring client information between agency facilities as needed.

 


Participates in the transfer of confidential client information and agency documents between area facilities.

 


Participates in the in-office holiday rotation to assure Admission Care Services on holidays.

 


Acts as liaison between agency and referral sources/facilities for the purpose of following current and potential clients and ensuring a smooth transition to home and SpiriTrust Lutheran Home Care & Hospice services.

 


Works to develop and promote good working relationships with referral sources with the goal of increasing referrals.

 


Processes and collects documentation to ensure agency compliance with Face-To-Face regulations.

 

Cooperates with, participates in and supports the adherence to all internal policies, procedures and practices, laws and regulatory requirements.

 

Works harmoniously with others.

 

Coordinates specific work tasks with other personnel within the department as well as with other departments in order to ensure the smooth and efficient flow of information.

 

Communicates with his/her supervisor, other supervisors, community relations coordinators and appropriate staff in order to integrate activities.

 

Responds to inquiries relating to his/her particular area, or to requests from other personnel, clients, etc. within given time frames and within established policy.

 

Maintains appropriate records and provides assigned reports.

 

Supports and advances the Lean philosophy in all endeavors.
