Education/Training: A high school diploma or equivalent; current CPR/AED/First Aid certification (or obtaining CPR/AED/First Aid certification within 30 days of starting the position).
Certified Therapeutic Recreation Specialist (CTRS) or Activity Director Certification (ADC) required.
Skill(s): Speak and understand English; moderate reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; valid driver’s license; may need CDL license in certain programs.
Experience: A minimum of (2) years of experience in a social or recreational program within the last 5 years, preferably in a patient activities program in health care setting

Attend trainings to build understanding and capacity to undertake QAPI work.
Carry out QAPI roles and responsibilities as assigned.
Assists with the presentation of daily life enrichment and special events by performing any or all of the following duties:
Assists with admissions activity assessments; carries out the resident’s care plan/support plan.
Maintains the resident’s medical record for life enrichment; records resident’s participation and attendance at activity programs.
Provides orientation to new residents concerning the life enrichment schedule and assists them in becoming familiar with different locations in the building.
Coordinates life enrichment when required; initiates and promotes participation in life enrichment within and without the community; promotes hobbies for residents, as directed by supervisor.
Recognizes changes in resident’s mental or physical condition; reports to supervisor or nursing supervisor in a timely manner.
Conducts outings and schedules trips; transports and escorts residents.
May assist in the production of the Activity calendar.
Changes bulletin boards on a regular basis, as required.
Monitors and documents residents’ food intake during activity events.
Assists in planning special events and parties for residents; coordinates efforts with volunteers; plans special trips as directed by supervisor.
Accurately records time worked or not worked as illustrated by the following:
Accurately records time in and out at the start and end of each shift and at the start and end of each meal break utilizing the Agency’s timekeeping system.
Makes corrections to the timekeeping system, as necessary, on appropriate forms within the required payroll timeframe.
Coordinates specific work tasks with other staff within the department as well as with other departments in order to ensure the smooth and efficient flow of information.
Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the work place.
Identifies, prevents, and reports actual or suspected abuse, neglect, exploitation, and improper use of chemical and physical restraints, in accordance with standards.
Cooperates with, participates in, and supports the adherence to all Agency standards, procedures, and practices in support of risk management and overall safety and soundness and compliance with all regulatory requirements, i.e., HIPAA, OSHA, etc.
Abides by and supports the organization’s code of conduct by exhibiting behavior that is set forth in the code of conduct. Performs duties in accordance with the code of conduct and completes annual compliance training.
Reports pertinent information to the immediate supervisor as requested, or according to an established schedule; compiles information as necessary or as directed and provides data to appropriate personnel.
Responds to inquiries relating to his/her particular area, or to requests from customers, other personnel, etc., within given timeframes and within established policy.
Completes educational requirements as mandated by state and federal agencies and to maintain competencies.
