Basic Qualifications


Education/Training: Possess a current unencumbered LPN License in the Commonwealth of Pennsylvania; current CPR/AED certification (or obtaining CPR/AED certification within 30 days of starting the position).  CPR/AED certification must be obtained by successfully completing “live” cognitive and hands-on skills evaluations through an approved course administered by an approved instructor.  Renewal of this CPR/AED certification must take place on or before the recommended renewal date on the holder’s card.  Online CPR/AED certifications are not acceptable.     



Skill(s):  Speak and understand English; proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communication skills; proficient decision-making skills; proficient assessment skills; thorough knowledge of current Department of Health regulations; demonstrated supervisory skills.


Experience: A minimum of one (1) years’ experience in related positions normally required.


General Responsibilities


Responsible for performing a variety of duties to provide quality nursing care to residents; performing various documentation and communication duties; ensuring safety and care of resident environment; providing residents with dignity and respect; achieving goals as established in the department’s annual operating plan and Quality Assurance Performance Improvement committee (QAPI); coordinating work within the department, as well as with other departments; supervising assigned personnel; reporting pertinent information to the immediate supervisor; responding to inquiries or requests for information.

Carry out QAPI roles and responsibilities as assigned.


Performs a variety of duties to provide quality nursing care to residents as follows:


Administers professional skilled services; assists with physician visits, physical examinations, and chart reviews; provides direct nursing care as needed.


Supports Complex wound treatments as needed.



Prepares and administers medications as ordered by the physician using current, accepted standards of nursing practice and quality assurance, as needed.


Ensures that Controlled substances are destroyed per protocol; reports any discrepancies to the Director of Nursing.


Receives physician’s orders for treatments and medications; acknowledges and handles orders promptly; delegates and prioritizes when appropriate.


Evaluates programs, systems, routines, and assignments for efficiency and effectiveness; recommends appropriate changes as needed.


Purposeful rounding on residents, team members to ensure safety of care provided.


Informs and ensures resident’s attending physician and family notification when there is a change in the resident’s condition or other pertinent information regarding the resident’s status.


Performs a comprehensive nursing evaluation upon admission and with any change in condition.


Performs various documentation and communication duties as follows:


 Maintains accurate and comprehensive resident information; charts progress notes on all residents in a descriptive manner.


Receives and provides verbal and written reports from prior to on-coming shift supervisors.


Utilizes the electronic health records dashboards and reports.


Ensures safety and care of resident environment as follows:



 Assures that all nursing work areas are kept clean, free of clutter, and are adequately stocked.


Performs all tasks in a safe manner, following established safety and infection control practices and procedures, including universal precautions and proper hand washing technique.


Reads disaster and fire emergency plans and elopement standard frequently, performs assigned responsibility in an emergency and assumes a leadership role.


Reports all incidents/accidents to self, resident, or others immediately; ensures that all reports are properly completed; initiates investigation, ensure an immediate intervention for safety and completes follow up as indicated.


Establishes an environment which promotes dignity and respect for residents as follows:


Prevents, identifies, investigates, and reports suspected or actual abuse, neglect, exploitation, and improper use of chemical and physical restraints, in accordance with Agency standards.


Knows and respects all resident rights, while maintaining the confidentiality of all resident care information.


Coordinates specific work tasks with other personnel within the department as well as with other departments in order to ensure the smooth and efficient flow of information.


Interacts with all colleagues in ways that assist one another in the successful performance of their duties.


Provides support, encouragement, and direction to the individuals under his/her direct supervision; acts as a resource for staff in problem solving and properly communicates within the established chain of command.


Creates an atmosphere that promotes a positive and respectful work environment.


Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the work place.


Cooperates with, participates in, and supports the adherence to all Agency standards, procedures, and practices in support of QAPI; risk management and overall safety and soundness and compliance with all regulatory requirements, i.e., HIPAA, OSHA, etc.; insures that the department and all personnel adhere to the same.


Abides by and supports the organization’s code of conduct by exhibiting behavior that is set forth in the code of conduct.  Performs duties in accordance with the code of conduct and completes annual compliance training.


Responds to inquiries or requests from customers, other personnel, etc., within given timeframes and within established policy.


Completes educational requirements as mandated by state and federal agencies and to maintain competencies.


Per PA DOH regulation in the SpiriTrust Lutheran skilled care buildings with 40 licensed beds, RN coverage on night shift (1030p – 7a) will be provided for the eight (8) hour night shift by an RN who is on-call and responsible to oversee total nursing activities (see below). As the only licensed nurse in the building on night shift, the Clinical Supervisor cannot leave the building unless another RN or LPN is available to cover for you.

Exception-Communities with a census under sixty (60) where an LPN is designated in charge on the night shift, an RN must be on-call and located within a thirty (30) minute drive of the center.
