SpiriTrust Lutheran is now hiring a LPN-Resident Care Coordinator
$3,000 Sign On Bonus 
Part Time = 4 shifts per 2 week pay period (30 hrs per pay) 

We provide residential living, assisted living, personal care, memory support and nursing and rehabilitation services in six senior living communities; home care, in-home support, palliative care and hospice services; an innovative program for clinically frail adults aged 55 and older in need of nursing home level-of-care; counseling services; financial education and coaching; domestic abuse solutions; services for the deaf and hard of hearing; volunteer income tax assistance; and an array of volunteer programs.

Your Experience Includes

Graduate of an accredited school of nursing with a current unencumbered LPN license


Current CPR/AED certification (or obtaining certification within 30 days of hire)
A minimum of one year experience in a related position normally required
Knowledge of State and Federal regulations, including Assisted Living regulations
Proficient assessment and organizational skills
Demonstrated communication and decision-making skills

Clinical Oversight and Administration


Oversees internal and external health related services and administers appropriate health care of residents in accordance with the Assessment and Support Plan
Facilitates communication and documentation related to resident health care
Supports, cooperates with and participates in the adherence to all SpiriTrust Lutheran standards, procedures and practices in support of risk management, compliance with regulatory requirements, and overall safety

 
Management Responsibilities


Oversees and manages Resident Care Partners, supporting the development and maintenance of a strong health services program
Provides guidance, support and encouragement to the care team with a focus on vision and purpose
Establishes an environment that promotes dignity and respect for residents and team members
Facilitates communication among the care team, supporting information sharing and problem-solving
