General Responsibilities


Responsible for performing various duties relating to the assessment process; performing various communication and documentation duties; coordinating work within the department, as well as with other departments; reporting pertinent information to the immediate supervisor; responding to inquiries or requests for information.


Essential Duties of an RNAC 

Supports the mission and values of SpiriTrust Lutheran which is: SpiriTrust Lutheran, witnessing to the Gospel of Jesus Christ, provides exceptional health, human, and senior living services by supporting persons in achieving an optimal quality of life.
Interacts with residents, clients, staff, and the general public in ways that demonstrate the agency’s commitment to courtesy, service, and hospitality befitting a church related organization.
Performs various duties relating to the assessment process as illustrated by the following:
Assures timely completion of the resident assessment instrument and related data entry within the guidelines of federal regulations. 
Schedules resident assessment instrument assessments and notifies interdisciplinary team of due dates
Coordinates development of the interdisciplinary plan of care.
Amends the care plan as required and communicates the changes to appropriate staff.
 Coordinates and facilitates care planning conferences.
 In regards to Medicare management, provides billing office with admission and diagnosis data, reviews medical records for ongoing skilled needs; highlights information for charting; notifies Social Services of Medicare termination dates.
 Provides assistance in regards to residents’ medical, physical, emotional, dietary, and spiritual needs and concerns. 
 Maintains forms and records according to federal, state, local, and SpiriTrust Lutheran standards and regulations.
Manages the restorative program for the skilled care center as illustrated by the following:
Oversees the planning process in setting residents’ measurable goals for the restorative program.
Implements residents’ restorative program and monitors compliance with goals.
Re-evaluates and updates residents’ restorative goals at least monthly.
Oversees documentation of residents’ progress in relation to goals.
 Directly supervises assigned staff as follows:
 Recruits, selects, orients, and makes provisions for the proper training of staff; regularly reviews the competence and performance of this group.
 Makes certain that all relevant information is properly communicated to the group in order to give them every opportunity to perform their duties properly.
 Organizes, schedules, and distributes work among assigned personnel.
 Conducts regular meetings with the staff.
 Works closely with the group in implementing an effective human resource management program to insure a competent, motivated group of employees.
 Administers established Human Resources Standards of Practice and other procedures as established by agency policy.
Ensures a safe and orderly work environment as follows: 
 Attends all required safety in-services; participates in all drills with a clear understanding of role in disaster routines.
 Ensures that assigned work areas are maintained in a safe, well-organized, and sanitary manner.
 Reads and understands role in all emergencies; assumes a leadership role.
 Provides residents with dignity and respect as follows:
 Identifies, prevents, and reports actual or suspected abuse, neglect, exploitation, and improper use of chemical and physical restraints, in accordance with standards.
 Knows and respects all residents’ rights while maintaining the confidentiality of all resident care information.
Performs various communication and documentation duties as follows:
 Assures that the care provided to each resident is documented accurately on the MDS in a timely manner, and in compliance with governmental regulations/agency standards.
 Maintains optimal accuracy rates of MDS as evidence d by minimal Utilization Management Review team error rates for nursing sections.
Oversees employees in their timekeeping process, ensuring accurate submission of data:
 Follows up with employees under their supervision to ensure timekeeping process is completed and submitted accurately with no errors.
Educates employees regarding requirements for timekeeping, missed punches, consequences, etc. 
Enters department’s timekeeping data accurately and submits data within practice guidelines.
Evaluates data related to resources and program and makes appropriate corrective actions with employees, if necessary, to ensure compliance with all rules, regulations, and agency standards.
Coordinates specific work tasks with other staff within the department as well as with other departments in order to ensure the smooth and efficient flow of information.
Abides by the current laws and organizational standards and procedures designed and implemented to promote an environment which is free of sexual harassment and other forms of illegal discriminatory behavior in the work place. 
Cooperates with, participates in, and supports the adherence to all Agency standards, procedures, and practices in support of risk management and overall safety and soundness and compliance with all regulatory requirements, i.e., HIPAA, OSHA, etc. 
Reports pertinent information to the immediate supervisor as requested, or according to an established schedule; compiles information as necessary or as directed and provides data to appropriate personnel. 
Responds to inquiries relating to his/her particular area, or to requests from customers, other personnel, etc., within given timeframes and within established policy. 
Completes education requirements as mandated by state and federal agencies and to maintain competencies.  

Basic Qualifications
Education/Training:  A graduate of an accredited program for registered nursing; BS in Nursing preferred; a current unencumbered PA license to practice as a Registered Nurse required.
Skill(s): Speak and understand English; proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; thorough knowledge of state and federal regulations; proficient assessments skills; proficient PC skills; proficient organizational skills; current CPR certification.  Refer to separate analysis for physical requirements of the essential functions with or without accommodation.
Experience: A minimum of two (2) years’ experience in related positions normally required.
