Telehealth Remote Monitoring Services
Daily monitoring of your vital signs can identify changes in your condition that might become serious. A telehealth monitor in your home allows you to do that monitoring and automatically sends the results to our nurse who knows when those small changes need further attention. This program uses innovative technology to check vital signs and monitor your health status — all from the comfort of your own home. The telehealth service is part of our home healthcare program.
How Does Telehealth Work?
Our small and easy-to-use in-home system asks a series of health-related “yes” or “no” questions that are easily answered with the push of a button. It also guides you through a simple procedure to collect vital signs including:
- Heart rate
- Blood pressure
- Blood oxygen level
- Weight
- Temperature
- Blood glucose levels
If any of the monitored results indicate a problem or potential problem, our nurse will call you and determine the next steps. It may include a visit from the nurse, a medication change or a trip to the doctor. The monitored data can be incorporated into trend reports and provided to your doctor on a regular basis.
This approach means gives you and your care team a chance to prevent a crisis that may otherwise lead to an emergency room visit or hospital readmission.
